Choose “save” on your browser and then choose to open the file in Adobe Reader or Acrobat (or you can save the file to your computer and open it separately).   Enter the information. Save the completed form and click on the link on the form to open an email message addressed to our membership chair.  Attach the completed form and click send.  THEN GO TO THE PAYMENT PAGE HERE.
